Archived consent agreements

Archived consent orders

Consent orders placed limits or conditions on, or suspended, a licensee’s practice where an investigation identified conduct or competence concerns and a citation was issued for a discipline hearing. Archived discipline orders

Discipline orders were made by the Discipline Committee after conducting a hearing to address concerns raised about a licensee. Examples of discipline orders included reprimands, orders for remedial educational activity, and limits or conditions on a licensee’s practice authority.
